Position Summary:

Joining our Care Management Division, we are currently recruiting for a Care Transitions Associate to join our team. The care transitions associate will join the care coordination team to ensure patients who have had an ER visit or hospital stay are provided with an immediate follow up appointment at one of our facilities.  Patients will be followed for thirty (30) days with the Care Transitions Associate providing timely contact with patients. 

Minimum Qualifications:

• Associates Degree in Medical Assisting, Medical Administration or related field of study is required, Preference for Bachelor's Degree in Health-related discipline. 
• 1-2 years of experience working in a community mental health or human services setting.
• Bilingual English/Spanish required.
• Excellent oral and written communication skills.

Key Duties:

• The CTA provides transitional care services for ER and Hospital admissions for 30 days post discharge.
• For patients consented to Bronx RHIO, reviews ER/Admission information, and documents pertinent Bronx RHIO. 
• Conducts follow up for admitted patients a minimum of two times within a 30 day time frame.
• Refers patients to Asthma Manager if the patient was hospitalized or in the ER due to Asthma. 
• Orients the patient and coordinates the initial primary care follow-up visit and reminds/encourages the patient.

Starting Compensation: Minimum: $48,000 annually; Maximum: $48,000 annually

Work Environment Type, Location, and Schedule:

The Care Transitions Associate will work intermittent Hybrid - working from home and based out of the Bronx (1 or 2 Wednesdays per month) working Monday - Friday 8:00am - 5:00pm and/or 8:30am-5:30pm.
